new obstetrics ob 3rd tri report!!!! improved n beyong WOW!!!!!!!!!!!!!!!!!!!!

PATIENT HISTORY:
A 27 years old Indian lady with G3 P1 A0 came for a 3rd trimester scan. Her menses was regular.
She does not have gestational DM and HPT.

LMP: 27/6/2009
EDD by LMP: 4/4/2010 (30w6d)
EDD by average U/S today: 29/4/2010
GA by average U/S today: 27w1d
Date of previous scan: 15/10/2009 (performed by doctor)
EDD by previous scan: 16/4/2010
GA by previous scan: 15w2d

Obstetrics Ultrasound Findings:
A viable singletone and intrauterine fetus was noted.
Fetal is lying breech and longitudinal.
Placenta is in fundal upper segment of the uterus.

Heart rate : 137bpm (within normal range)
AFI: 16.09cm (within normal range)
EFW: 1129gms
Umbilical artery RI: 0.70
Cervix length: 3.76cm



Growth scan done.
EDD by U/S today correlates with EDD by previous scan.

WSDE

Patient history:
A 64 years old Indian male came for a KUB scan.
Patient had bilateral posterior falnk pain for 1 week.
Patient's urine test 3 days ago revealed microscopic hematuria.
Patient is on analgesics for the pain as well as medication related to a coronary bypass surgery performed 3 months ago.

U/S findings (KUB):
Right Kidney:
Right kidney appear normal in bipolar length and homogenous texture.
Contour appearance was irregular.
Corticol medullary differenciation is still preserved.
BPL length: 9.38 cm
Parenchymal thickness: 1.09 cm

Left Kidney:
Left kidney appear normal in bipolar length and homogenous texture.
Contour appearance was regular.
Corticomedullary differenciation is still preserved.
BPL length: 9.79 cm.
Parenchymal thickness: 1.25 cm

Bladder:
Bladder volume was 184.03 ml and post void volume was 25.15 ml.
Bladder jets were visible.
Patient is nil by oral (didn't drink) due to Muslim fasting month.
No other abnormalities noted.

Prostate:
Prostate volume was 6.87 ml.
Prostate appears normal in size.
Prostate contains echogenicity in the medial region indicating

Patient is a 32 year old Malay woman with G1P0A0 coming for a 1st trimester scan.
Patient does not have gestational diabetes or hypertension.

Findings:
LMP: 16/10/2009
GA by LMP: 12w16d
EDD by LMP: 23/7/2010
GA by U/S: 8/4/2010
EDD by U/S: 22/8/2010

GS: 3.29cm (8w6d)
CRL: 1.79cm (8w2d)
Fetal HR: 174bpm
Yolk sac seen.

Viable singleton seen in the uterus.
Decidual sign was seen.
No abnormalities of the uterus, ovaries, adnexa or kidneys were seen.

story n question

One gets used to the rain. The human mind can adapt to many things given the time. Being born in Forks, I never felt the gloom associated with the dreary weather like many of the other locals or visitors. I never saw what was so bad about living in a place that had rain more than 300 days a year.
Today, however, was different.
Panicked thoughts surged through my brain as I struggled to find the words I wanted to say. Don’t leave. Please stay. I need you. Do it for us. But I stood there speechless as Renee packed the last of her bags an walked to the door.
“Don’t worry about your money,” she said through poorly disguised tears. “I won’t ask for any of it. My lawyer said we can make this as amicable as possible.”
But I don’t care about the money.



Rumack et al are quoted to saying that a detailed examination of the foetal neck is not a standard part of the ultrasound examination of foetal anatomy.

However, certain abnormalities, masses that appear in the region of the neck can be significant ultrasound findings. Abnormal findings of the neck will in most cases require careful patient counseling and management
